(Answer) (Category) FreeSurfer Faq-O-Matic : (Category) File Formats :
How can I make and process my own label files?
The label file format is used by both tkmedit and tksurfer as well as many other binaries. It can describe points in the volume and vertices on a surface. Each vertex/voxel in the label has its own line, and consists of a coordinate and an optional vertex index. If the entry exists in the volume but isn't on a surface, the index is -1. Here is an example of the file format:

3
72130 -45.480 -16.660 46.630 0.000000
72129 -44.900 -17.000 46.960 0.000000
72150 -45.700 -16.710 46.130 0.000000

The first number is the number of entries in the file. The first number on an entry line is the vertex index. The next three are coordinates in RAS world space (x,y,z). The last two values are option and are used differently by different programs.
